Section 17-A. Notification

The notification required upon receipt of an application for a license pursuant to sections fifty-four, fifty-four-a, fifty-five, sixty-three, sixty-four, sixty-four-a, sixty-four-b, sixty-four-c, eighty-one and eighty-one-a or for a renewal under section one hundred nine of this chapter shall be made in such form as shall be prescribed by the rules of the liquor authority. The liquor authority shall require such notification to be on a standardized form that can be obtained on the internet or from the liquor authority and such notification to include: 1. the full name of the establishment; 2. the full name of the owner and/or applicant; 3. the address of the establishment, including the floor location or room number, if applicable; 4. the name, address and telephone number of the attorney of the applicant; 5. a line indicating whether it is a new establishment or a transfer; 6. if the establishment is a transfer, the name of the old establishment and such establishment's previous license number; 7. if the establishment is a licensed facility, the license number of such establishment; and 8. the type of license.
