Section 4138-A. Certificate of birth data

1. A certificate of birth data shall be made whenever proper proof is submitted to the commissioner that a person, who is under the age of eighteen and born outside of the United States has been validly adopted by residents of this state pursuant to a judgment, order, or decree of adoption issued by a court of competent jurisdiction in this state, another state, or a foreign country. 2. A certificate of birth data shall contain such information and be in such form as the commissioner may prescribe. The sufficiency of the proof submitted shall be in the sole discretion of the commissioner.
