The department shall submit a report to the governor and the legislature on or before the first day of January in each year in which contracts under this title are in effect, on the effectiveness of reaching the goals and objectives sought under the provisions of this title. The report shall review all community service projects funded under this title. Such report shall also include the number of pregnant and parenting adolescents and at-risk youth and families served by project, by age, and by services received, a description of specific indicators of success or achievement obtained by persons served by each project, including the number of such persons who obtained employment, continued in or completed high school or an equivalency program, or completed a job training program, and to the extent possible, the effect of each project on the incidence of adolescent pregnancy and on indicators of risk of adolescent pregnancy within the service delivery area of each project, including a comparison of the total number and rate of pregnancies to persons aged twenty-one and under prior to the start of each project and during the twelve months preceding the due date of the report and the incidence of second pregnancies to adolescent mothers served by each project, the number of pregnant, parenting or at-risk youth who were eligible for, able to receive and benefit from county and state public assistance programs and the results within each community of efforts to increase the overall coordination and effectiveness of services. Community service projects shall provide information to the department necessary to prepare the report.
