Section 28-304.6.5. Inspection and test reports submission

Inspection and
  test reports shall be submitted on such forms  and  in  such  manner  as
  required   by  the  commissioner.  Copies  of  each  report  (each  copy
  originally signed by the inspector performing the  inspection  and  test
  and  by  the  inspector  witnessing  the inspection and test, the agency
  director, and the building owner) listing all violations of any  of  the
  provisions  of  this code for each device inspected and/or tested, shall
  be delivered to the owner within 30 calendar days of the  inspection  or
  test  and  filed  with  the  department  within  45 calendar days of the
  inspection or test.
